Case Management
Striving for Excellence
Meridian Services has contracted with counties throughout the state of Minnesota to provide Case Management to persons served with developmental disabilities, mental health, chemical health, elderly physical challenges, and related conditions. The goal is to provide high quality support services and to assure that needs of persons served are being met.
Our Case Managers perform the following requirements: needs assessment; individual service plan development; screenings; coordinating team activities; assisting with planning for new service development; locating residential, vocational, and other needed services; monitoring service delivery; conducting persons served visits; developing waiver budgets; and acting as advocates for persons served.
Depth of Experience
Meridian Services Case Management Department is a team of experienced professionals who work collaboratively, sharing their experience, knowledge, and insight. The Case Management Department is supervised by a Licensed Social Worker who is also one of our company’s Executive Administrators. Our Case Managers perform all requirements of Rule 185, VADD, Waiver Case Management , as well as the Alternative Care Program.
A Viable and Economic Option
Contracted Case Management Services is a viable and economic option for county human services departments. Cost savings include direct and indirect expenses, including the cost of transportation, maintaining office space, and the expenses the county incurs for training and providing benefits for their own employees.

Our contracted Case Managers have become familiar with many of the cities, towns, and rural areas that comprise the communities of Minnesota. We are knowledgeable of many of the resources available to the person served, but also have experience locating resources in communities that are new to us. We have contracts in the following counties and healthcare systems:
