Meridian Services' Support Coordinators are experienced professionals who work collaboratively, sharing their experience, knowledge, and insight. Support Coordinators work with families, Case Managers, and individuals with disabilities to carry out the essential elements of the Individual Service Plan. Duties may include assisting families in the development of an Individual Services Plan addendum, or a Health and Safety Plan, assisting families in identifying and acquiring needed resources, and facilitating person centered planning. Support Coordinators may also monitor plans, and assist with plan revisions. Support Coordinators communicate with families and Case Managers to ensure integrated planning.

Meridian Services contracts with counties to provide Support Coordination services to consumers with developmental disabilities and related conditions. The goal is to provide high quality support services and to assure that needs of individual consumers are being met. In addition to Meridian Services' experience providing Support Coordination services, we are experienced in person-centered planning using Essential Lifestyle Planning (ELP) and Planning Alternative Tomorrows with Hope (PATH). An affiliated company, Orion Intermediary Services Organization (ISO), is a provider of Fiscal Intermediary / Employer of Record services. Orion ISO was one of the first companies in the state to develop the Consumer Directed Community Supports (CDCS) option.
