Service Coordination for Older Americans
We provide older Americans and their families with access to the resources, services, and support they need. Our Service Coordinators are experienced professionals who work collaboratively with older Americans, their families, other professionals, and service providers to access the services needed.

It is often the case that family members are unaware of the range of services that are available to older Americans. In addition, it can be an overwhelming task to arrange for and to coordinate these services. This is the role played by our team of Service Coordinators. They are very familiar with the wide range of services available to older Americans and take a “hands on” approach, assessing need, researching programs, and empowering older Americans and their families to make informed decisions.

We have a skilled team of caring Service Coordinators with the education and knowledge needed to coordinate these tasks and services for older Americans and their families. Our Service Coordinators work closely with professionals who are experienced in the areas of medical and psychiatric resources, legal and financial assistance, housing, transportation, recreation, educational and vocational opportunities. We are able to assist older Americans and their families in navigating complex systems and completing the paperwork involved with various benefits, including Social Security, Medicare, and Veterans benefits.

Service Coordination is an economical option for families, consumers and their legal representatives who desire program or other technical support to facilitate the use of the funding resources available in the community.

A History of Quality
Meridian Services was founded in 1980 as a company dedicated to serving people with disabilities. We began to provide Case Management Services in 1984, and expanded that service to include Support Coordination in 2001, under a consumer directed program for people with disabilities.
